Someone has gained access to my PC and Android smartphone

My ex boyfriend has gained access to my PC and my Android phone, he is in my Facebook, email, SMS, ect. He had used some of my contacts and wrote to me and call me, just like I was them. If I'm with someone, their phone rings and it's my ex!  How does he do that?!  My phone rings and I see the number, it's my neighbor, I answer call and its my ex.  My phone flashes like crazy all these different names and numbers, not allowing me to gain entrance to it.  He is getting my texts, emails, contact numbers, their texts to me and emails.  It's driving me crazy, is there anything I can do to stop this adolescent behavior and not to mention breech of privacy and a huge waist of time that I could be concentrating on my things I really should be doing?

Software/Hardware used:
I'm on windows 7, Facebook, Gmail, Verizon, Chrome

First thing I would do is from another computer reset all your passwords. Then run a virus check on your computer and see if there is a key-logger installed that may be allowing him access. As for your phone, there are programs and app that will let you spoof another phone number. If you still have problems report back with more info.
