I'm curious as to how often a company's security policy was checked on average? Is it yearly, quarterly, etc.? Does it matter if there has been security incidences, or does it vary?

    Many (most?) security policies are written by IT professionals and stuck in a notebook or on a network share never to be seen again.

    A reasonable way to manage security policies would be to review them on a periodic basis, i.e. during an annual risk assessment or security audit. It just depends on your business requirements.
