How can I accomplish internet service to 450 locations with each location having approximately 50 users over a single service provider?

What I'm trying to accomplish is a "single" internet service to 450 locations with each location having approximately 50 users. The connections has to be over phone lines (DSL) or wireless (4G) What would be my best option?

    Impossible to say without knowing the 450 locations. With 50 or so users in each, it's likely that each location has multiple choices; but we can't know if a single vendor serves them all.

    E.g., my location has exactly one DSL or better ISP. It's large enough to cover many U.S. regions, but definitely not all.

    Years ago when I had DSL there were limits. You had to be within a certain distance from one of the providers wiring stations. And it was a short distance, i think less than 2 miles. The further away from the station you were, there was some loss in bandwidth. Check with you local DSL provider, they may have the answers for you.. you mention it has to be over phone lines, is there a specific reason you cannot use cable broadband? That would be much faster.
