    First problem you have is looking for anything useful at all for an "AS/400". While AS/400s were advanced and extremely capable in their time, they've effectively been obsolete for most of this century. It's quite possible that you're using the term because it's a name that has stuck, but the platform has evolved a lot since IBM stopped selling that name back in 2002 or so.

    Searches done today should be for "IBM i" rather than "AS/400" -- unless you are actually using an older AS/400 model. And if that's the case, you'll need to be very clear on what model you have and what its OS version is.

    Next, are you looking only for "patient scheduling"? That's a pretty limited part of any medical clinic/hospital/office package. It might be difficult finding any package that doesn't cover much more.

    Do you have IT staff or would you willingly bring in an outside resource? That might be important for some types of software. For example, ClearHealth is open source and has a scheduling component. It might be possible and reasonable to install it and use only the scheduling.

    ClearHealth isn't a traditional "AS/400" application, though, and might not even be possible on the older versions. But if you actually have an iSeries or, better, an even newer version of the platform, it should be about as easy to install and use as it would be to purchase other hardware and implement there.

    As open source, it's possible that some other clinic or agency has already done something similar to use only limited components of the whole package.

    Regardless, you'll need to work up a much more detailed description of the system environment and the requirements of the software. Otherwise a 'recommendation' will be worth about as much as one costs.
