File->Application->New.  Leave the server as Local.  Give the DB a title.  Pick a location and file name if you want.  If there’s a particular type of DB you want, choose the appropriate template.  The rest is all optional, and should be fairly self-explanatory.  Most of the advanced items are just for tweaking, and can be changed after the fact anyway.
