Good advice Yusuf. Something else to think about though - most of these are symptoms of conflict avoidance - not having clear open communication early on and along the way about reward and recognition and mistakes; holding people accountable for deadlines, etc. Managers can set the stage for constructive and creative conflict early on in the project to avoid destructive conflict down the line.
