WSUS
I have a server 2008SP2 with WSUS v3.0 SP2. I would like to be able to rely on this for reporting and updates. The problem I am encountering is that not all my servers are showing up in the WSUS console, so my reports are not accurate.
If I go to a "missing" server and run wuauclt /detectnow, then that server will show up in the WSUS console. However sometimes it will subsequently disappear again later.
Anyone know why this happens and how to prevent it so all servers show up all the time? All our servers are in the same OU with the same WSUS GPO, so I do not understand why this is happening.

I found the answer to this. It has to do with a registry entry. If the server is created using a template, such as our vm servers, or was cloned with cloning software, then we have to go delete two registry keys. This is because the keys already exist on the original server and are cloned as is to the new server. They have to be deleted.

The keys to delete are SusClientId and SusClientIdValidation.
Next I delete the server names from the WSUS console.
Once these are deleted then I ran wuauclt /resetauthorization and wuauclt /detectnow. The keys are automatically regenerated in the registry and the servers show up in WSUS and don't "disappear" any more.

I’ll check the event logs, thanks. But I can also tell you one day I ran the detectnow command on three servers not showing in WSUS, They showed up finally, but then within hours were gone out of the console again and noone manually deleted them. So it does not fit either scenario you offer and that is what has been so frustrating!
Are Network Discovery turn OFF? Sometime this may cost the problem.
I did check the event logs and did not find any errors pertaining to the disappearance of servers from the console.
Network discovery is not turned off, no.
What about Sysprep? I tend to sysprep a machine or VM before deploying and I’m curious if that would clear up your problem as well.
I find that our Anti-virus and inventory tracking applications report similar issues like your WSUS if we don’t sysprep our images ahead of time.
