I like the Idera SQL Diagnostic Manager for monitoring SQL Servers, especially in an enterprise environment with lots of servers.
I will let others comment on SQL diagrammers - there are several available, and they all have their pros and cons.

Take a look at Quest Software’s Spotlight on SQL Server Enterprise. It is very inexpensive compared to Idera’s Diagnostic Manager. As for diagramming, use the built in diagrammer that comes with MS SQL.
