If I report e.g. one second of CPU time used by Windows running as a guest under ESX how much CPU time is reported by ESX?  Is it more, less or the same?

It should just just slightly more. There will be the one second of CPU time, plus the tiny bit of overhead which it takes for the host to control the guest during that one second.
