yes, we did upgrade the folder design with her client and template at R7, same issues occurred. before that we had deleted the cache.ndk but it still happened. We are thinking of uninstalling and reinstalling.

Has a Notes administrator run fixup, compact, and updall on the mail file? These three utilities solve 80% of notes database problems.
