Where can I find network diagrams of Layers 1-7?

Where can one find Layer 1, Layer 2, Layer 3, etc. network diagrams? It seems that different companies have different ideas of what should be displayed in each diagram. Is there a uniform book with clear samples that one can follow?

Tool for mapping unknown Network/Data ports?

Two recently acquired offices lack network diagrams and there is nothing to show which data ports are live or how anything matches to the patch panel. Besides manually visiting dozens and dozens of ports with a laptop -- what are the options? Surely there is a tool for this scenario. Would someone...
