Is mail routing ok? it sounds like your mail.box is corrupt. You can down the server,rename the server mail.box to mail.old and restart the and check if the problem persist.

There are logs and many other things you can start looking at but if you want everything and if you want to have it all organized and easy to understand then you almost need to use IBM’s Tivoli for Domino. They charge about $650 per processor but I think it is a very good tool.
