We use a spam filter provided by MX logic, and it is the best filter that I have seen in the last 6 years. We had tried several over the past 6 years only to be dissapointed. We finally found MX Logic and Spam is no longer an issue, it is pricey but in my opinion worth every penny. We have used it for the last 2 years with no issues whatsoever .

The way it works is all mail goes through their filters first prior to being delivered to our exchange server. If it finds any Spam it quarantines it and then sends an email with a link to the quarantine so you can decide wether or not to release it or not.
