5 pts. SAP BW transports best practices/guidelines/standards?

I am looking for some standards/suggestions/guidelines/best practices for maintaining transports in SAP BW. Here is a problem that we had and are trying to avoid in a future:We made a change (A) in Dev and moved it to Q. While waiting for the client to test and approve the change, there were some other changes (B,C,D) made and transported from Dev to Q to Prod.
When the change (A) was finally ready to go to Prod the transport moved old versions of some objects and overrode the changes that were made by transports (B,C,D).
How do others deal with this?
