    Ur Q did not clear all the points..... what did it mean ! "tables are used in a membership database other than name & address", have u tried to create database named as "membership" for any club/associations purpose ! where name/add r not required but other infos r required !! Not cleared here. If u considered from a template like this then simply delete the field which u not want. However, these all r my guess. If u want to add/delete plz click here/ for relationship tables and for use template
