Provide outside access in a workgroup
My wife has a similar requirement (also under HIPAA), and Sharefile.com was a good solution. With audit trails, it is easy to find out who accessed files and when, even receiving alerts for file uploads and downloads. They offer a Sync feature that could help you manage the external aspect.
