When organisations have different hardware and software environments, how should they ensure that both organisations’ systems ‘understand’ the message being sent between them?

To ensure that the message is "understood" by software on both platforms you should send the message using a non-proprietary format such as XML. If you must use proprietary formats because that's all the systems allow you can use software such as Microsoft BizTalk as a "router" to convert the data from one format to another.
A lot of user education and reinforcement helps too. We have an ongoing battle with Mac to Windows and different versions of Office.
