    Several factors might be for this issue. How did you run this form like locally to remotely? How could you sure that the system is configured properly? Have you properly modify tnsnames.ora after the installation according to the system's environment? Moreover, there are two Java Runtimes which are supported by forms builder, while Oracle Jinitiator is used by forms builder by default.

    At least you should have provided information about:

    -The forms version you are using
    -The browser you are runing the form on
    -The operating system
    -The error message you get (if you get one)
    -How you are trying to run the form
    -Any configuration changes have made/tried
