Need help on Lotus Notes/Domino development

I have developed a training database that is a repository for our training materials. I want to add a feedback form so users can send feedback on specific documents. My thoughts were to add an action button for feedback. I want it to open a form, populate the user name, attach a doclink, provide a comment field and a button to send the email when finished. I can get the feedback button to open the feedback form. However, I am unable to attach a document link. I have not started to think about how to code the send yet. Any ideas are appreciated. I am a novice programmer, so please keep answers simple. Thanks,

Do you want to provide a field where a document can be attached to on your Form or when the form is saved/submitted do you want to provide functionality that will send you an email with a document link to the Form/document?

    I'd prefer to create a new form with a link to the first form. Once the user completes the feedback form, click a button to send an email with their comments and doc link to the original doc.For example, if user is reading Notes Training document and finds an error; click feedback form; add revisions; click send. I get an email with their comments and a doc link to Notes Training document.Thanks
