You do not mention what version of exchange you are using. Please include details when you ask questions. We can answer with more detail when we know what you are using.
When a user changes their last name you do not need to create a new mail account. The best thing to do is change the last name on their existing account, then add a new SMTP address and make it primary. You should keep the old SMTP email addresses so that they can still receive mail from people who do not yet know the name has changed. The details of how to do this depends again on what version of Exchange you are using.

You didn’t mention what platform you’re using, so I’ll assume Exchange and add a few tags to let others find your question. Tags are used to properly sort your question to get it in front of those who can answer. For example, if your question is about Exchange 2003, then use “Exchange 2003″ as a tag, etc. Thanks!

For Exchange 2003 the SMTP addresses are edited using Active Directory. Go the the properties page of the user and click the email addresses tab. Add the new email address with the new last name and make it the primary SMTP address. You will also want to change the last name of the active directory account itself of course.
