Just Restart the synchronization service

Are you downloading to a PST? If so, keep in mind the PST is a local file and changes you make to it do not replicate back to the server. The server will notify outlook new mail is available but the client does not notify the server when email is deleted, read, etc. Communication is one way from the server to the client when you are using a PST.

What do you mean by out of synch? Is there a few minutes delay between sending and recieving?
