Look at the list of forms in the Designer client.  On the right are 2 columns.  One has a Notes icon at the top and a check mark in that column indicates that the form is available in the Notes client.  The other column has a globe at the top and a checkmark in that column indicates that the form is available in a browser.  Is there a checkmark in the column for web access?
