To be able to aswer :
1/ Lotus domino version used : 6.5x, 7.0.x, 8.0.x , 8.5 ?
2/ The forms version use : 6 , 7 or 8.

check that the signiture file is accessable… etc but “notes item not found within Inotes” normally points to a design issue
