How to dig out ” lost ” workflow in SAP

One workflow was checked, and forwarded to upper level manager for review and approval, mighted be not released from " reserved " . both forwarder and upper manager can not find this workflow in their SAP Inbox. even SAP prompted this wrokfolw was reserved by upper manager, and in forwarder's outbox, this workflow show it's status as " ready", anyone can help on solution how to dig out this workflow, it should not be really lost, but we don't know where it is.
