Cluster Shared Volumes are created via Failover Cluster Manager (we will actually be posting a video on this soon), but to do it, you first need to have a cluster, then add a Disk to the cluster (initialize, and give it a drive letter through disk administrator), then add it as a disk to the cluster, and then add it as a Cluster Shared Volume. It isn't too difficult, once you get the first steps down. Check out this blog: http://blogs.msdn.com/clustering/archive/2009/02/19/9433146.aspx

To setup a cluster on Windows 2008 you have to present a shared disk over iSCSI using something like StarWind Software's iSCSI target. I just published a whitepaper today which says how to use there software to do this. There's also a webcast coming up next month where I show how to do it.
