We use Avocent DS View. It allows us easy management of both our physical and virtual environments.

Our company uses VMware vCenter Server. I find it very effective and better than most other ones i have tried.

The standard tools work great for our environment. For VMware, VirtualCenter Server is good for virtual servers. We use Remote Desktop and VNC for physical servers. Also the MMC for Active Directory and Exchange management.

That depends on what you mean by “manage”. Are you asking about a monitoring system? Are you asking about management consoles? Can you give us more specifics about what you are using for vm and what OS is on them and your physical servers please?
