There are a couple of ways to match disks to LUNs.
1. Your multi-pathing software should be able to match up disk id with LUN.
2. Your storge admin tool sould be able to tell you what drive letter is assigned to the LUN after you assign the LUN to the host.
(I'm coming from an EMC point of view, your storage playform may be different.)
In powerpath it will give you the disk id, and over to the right it will give you the LUN information.
In Navisphere after the LUN is assigned to the hosts, refresh the pain on the right and navigate back to the host. Next to the LUN it should show the drive letter assigned by Windows.
Within diskpart, after creating the partition using the CREATE PARTITION command use the format command to format the disk.
