Doubt in view


Lotus Notes 5.x

A view has been created and its extracting values from the form.
In the form there is only one entry for ex : CSISDate (field) is there.But while in view i can see CSISDate,CSISDate_1,CSISDate_2,CSISDate_3,CSISDate_4.I didnt understand how did i get all these names.Is there anothing like changing fieldnames soon after we enter the values in the form.Please kindly help me on this issue.Its really urgent.
