What database version are you using ?
When you install Forms, a file called “tnsnames.ora” is created. In this case it should be under “D:\orant\NET80\ADMIN”. In this file you define your database connections (database server name or ip address, protocol, port, service name/service identifier).
Here is the basic syntax of tnsnames.ora:
When you connect from forms to the database, you should use net_service_name as “Database”.
For example, if you tnsnames file looks like this:

In forms, you connect using TEST_DATABASE as “Database”.
