Are you doing automated forms?

Or are you talking about creating the forms then printing them out and having them filled out by hand?

If you are print forms, just make them in Word, or a similar text editor, convert to PDFs and then print.

If you are making them online or on system, run a java applet and log into a text file or something like that.
