Clicking 'save' Notes Web application creates duplicate copy


Lotus Web applications, Lotus Notes

We have a Web-based application in Lotus Notes. I have created a new form to add some details. When I click on the "save" button, sometimes a duplicate copy of the same document gets created. How can I avoid this?
