We're using the "same" Barracuda spam filter that Willybst mentioned. From time to time (usually right between updates, or a down spam server), we might get a blast of spam. For the most part, spam is a very-very-very small issue.

We use externally hosted vendor Message Labs to do all our SPAM filtering. Taking care of it ourselves was becoming a full time job! They guarantee 95% reduction in SPAM and 100% blocking of email viruses. Well worth it to us to let them do it since we have 12000+ email users!

We started out with Symantec email antispam. However, it was expensive per seat. I moved us to Sunbelt Software's Ninja and it works pretty well. I like the way it dumps the spam into the user's Spam Quarantine box and lets the user decide if it's spam. It helps cut down on the false positives.

I would recommend you check out [A href="http://www.securence.com/"]Securence[/A] You can then limit your firewall to accept email from Securence's IP ranges. There are other hosted filtering companies, but I have had great support from Securence.
FWIW, YMMV

We are using an externally hosted Barracuda spam filter (http://www.tcnoc.com/) which works great for us. We used to use Antigen until the end of 2007 but it was a constant headache and sometimes a nightmare. Now I feel we don't have a care in the world, so to speak. In part an onsite solution might feel more secure but with some of the spam filtering of Exchange 2007 and Outlook 2007, we're running smoothly.
